Community Mental Health

Supports people with a mental illness to live well in their own home and community. Programs include regular visits and support from a mental health support worker and group activities where people can meet other people, learn new skills and link into their local community life including looking at recreation and employment possibilities.

We aim to positively support each person as they learn more about the effects of mental illness and its management in their own unique situation by providing a person centred care approach.

In all components of the Community Mental Health service, there is a choice of pathways for the consumer to assist in their recovery. The support is planned and provided using an individualised, personalised and consultative approach with each individual and appropriate relevant people such as family members, close friends, case managers and significant professionals.

All work is guided by support and recovery plans within the recovery star model of care. This incorporates clearly identified goals and objectives of the person, including seeking employment.

All plans are designed to be consistent with the principles of psycho-social rehabilitation and clearly identify time lines and processes for concluding support. We encourage recovery following a mental illness; promoting community involvement, participation and acceptance.
